ip was working on a 20ft wall form installing wing nuts on walers to secure the formwork ip was working with another coworker that was feeding material from the bottom using a rope. Ip had finished working on an area and was shifting to another area while on a wall ip removed his lanyard from the anchor point as he was moving ip slipped causing ip to fall approximately 10-12 ft to the concrete ground
