What happened
The EE was standing in a rack that is used to place trays of food on and transport from one area of the plant to another via a double long forklift. The forklift operator could not see the injured EE standing in the rack so he drove into the rack to pick it up to move it when the double long fork struck the injured EE 's left ankle.
