Moving slab from a-frame to cnc machine with suction lift

Took lift suctioned it to stone top picked up 1-2 feet off aframe spun it 180 degrees centered it laid it flat 90 degrees. Moved it over table that’s when the stone split horizontally in half and fell off lift straight towards emploee. Remaining piece dropped from lift pinning employee between it and aframe
