What happened
On [REDACTED] around [REDACTED] a team of production operators and mechanics were cleaning up a bottle crash at our [REDACTED] labeler. After identifying that there may be water in the glue pot and noticing the sliding door to the chute was slightly opened a mechanic went to close the chute completely. At that time as the mechanic backed away the team heard a pop and the glue erupted out of the chute (traveling approximately 12) ultimately reaching the mechanic and resulting in injury burns.
