All of the employees including [REDACTED] were working at the pita packaging line were at the beginning of the conveyor removing bread from the belt as the bagger machine was being worked on.

Since the bread had to be removed from the conveyor belt there were stacks of trays nearby to place the bread in. There was a single tray placed behind where [REDACTED] was standing. He went to turn around and walk away and this is when he tripped on the tray and fell to the floor landing on his right side injuring his shoulder.

The employee felt pain his his right shoulder. He could not move his arm up more than 70 degrees.

The single tray that was on the floor directly harmed the employee.

Employee was at the conveyor with the rest of the employees from the line removing bread. He went to turn around and tripped over a tray. He landed on his side and injured his right shoulder.
