The employee was at the warehouse outside of the baler machine room. The employee went to go retrieve a red barrel and a container to discard cardboard.

As the employee was walking outside the baler machine room he slipped and fell on oil on the floor. The oil came from the totes brought into the baler room to be discarded. When the employee fell he hit his head on the floor injured his shoulder and felt pain in his waist.
