Before the incident
The pine bleach plant was shut down with the machine outage to replace a bad valve at [REDACTED] tower The operator who was doing the LOTO on the [REDACTED] MC Pump opened the dump valve before he had closed the suction and discharge valve on the pump Because of this the contents of the preretention tube began dumping out around him and he was exposed to some amount of residual ClO2 still present in stock water that was in the tube As the morning went by he noticed he was having chest tightness throat burning and difficulty taking a deep breath Employee sought medical treatment beyond first aid
