Scraping changing over line 4 to different product
What happened
BPO was performing the task of removing the augers and scraping the dough from the pretzel extruder on line 4 for a changeover. The task was being completed in between the extruder and the proof belt. When the equipment is in operation the extruder and the proof belt are connected by a ratchet strap. During the changeover the strap was loosened to move the equipment away from each other. The BPO walked between the equipment to scrape the extruder and when she went to walk out of the area she forgot the ratchet strap was there. She tripped and fell fracturing her right hip.
