What happened
EE was releasing the trailer from [REDACTED] at [REDACTED]. EE thought the load leveling place was fully retracted and began to walk around to the other side of the door to action the chains for the overhead door and tripped over the dock plate and fell onto the floor landing on the left knee and left elbow. EE recovered and attempted to return to forklift but felt discomfort in the back and contacted management
