Avonelle Combs took a full tray out from incubator. She sprayed each side of the tray twice with [REDACTED]. She walked to the Biological Safety Cabinet (BSC) and place the tray inside. While she was turning to go back to the incubator she slipped and fell on her knee. She stated that her whole leg went in one direction while her knee went to the other direction.
