Working on the drill press

Employee was working on the drill machine to drill holes on a round metal plate to make a drain cover. He had used the vice grip but believes he did not secure it tight enough because he lost control and the drill made the plate spin cutting the top of hand between his thumb and index finger. Cut was about 1.5 to 2 inches long.
