Employee climbed into the back of the still running Accu[REDACTED] [REDACTED] to utilize the dust collection suction tube to smoke her vape pen undetected in the facility.

What happened

Employee climbed into the back of the still running Accu-System [REDACTED] to utilize the dust collection suction tube to be able to smoke her vape pen undetected in the facility. The employee crouched down near the floor where the dust collection suction tube was located; and began to smoke. Directly above the employees head was a drill bit spindle that was still running since the employee failed to power down the machine. Right above each of the drill bit spindles is a smaller dust collection tube. As the employee was crouched down smoking her vape pen; the smaller dust collection tube above her head started to pick up strands of her hair. This caused a significant amount of the employees hair to become wrapped around the still functioning drill bit spindle. This resulted in a 5 in diameter section of the employees hair and scalp to be removed from her head and entangled in the spindle.

Injury or illness

Employee was scalped. About a 5 in diameter section of hair and skin was removed from the top of the employees head.
