[REDACTED] came to do a bedside debridement. Scalpel was left open in the tissue tray after use by [REDACTED] and was covered with a gauze pad. LPN reached into tray to remove gauze prior to discarding tray and contents in humipak bag. She did not see that a scalpel was under the gauze and was cut.
