Before the incident
Employee was packing flour tortillas since the start of shift. When employees were asked to rotate employee climbed the steps of the crossover ladder to get to the other side of the production line and employee fell as he was descending on the stairs.
What happened
As she is descending on the crossover ladder employee lost her footing and slipped and fell.
