On [REDACTED] employee reported a sharp lower back pain and she found it difficult to walk bend stoop and twist. She stated that her muscle spasm and pain happened after bending to try to lift the pallet the day before on [REDACTED]. On [REDACTED] her pain continued and worsened after she had been working at [REDACTED] area for 1 hour.

Diagnosis: sprain of ligaments of lumbar spine initial encounter.

Improper body posture as she tried to bend to lift. Did not lift. Strain to lower back by bending.
