Moving a wooden crate containing a glass door onto a vertical stack on a storage rack.
What happened
The employee was adding a crate to a stack of doors on a storage rack. The employee was unaware that an employee from the previous shift removed the tilting device used to stabilize the stacks from the back of the rack. When the injured employee pushed the door onto the stack this caused the door to become upright and fall forward pushing the employee back and causing a fall.
