What happened
EE was utilizing a manual pallet jack to take out combo waste. The waste was on a pallet. EE was pulling the pallet jack through the double doors towards the incline. EE lost control of pallet jack and let go of the pallet jack. The pallet jack came towards EE and caught EE fight between pallet jack and concrete wall at the incline.
