What happened
The employee was operating a fuel tanker driving from the fuel farm to the GSE fuel farm. While maneuvering the vehicle the employee 's pen slipped and fell into an open vent inside the truck cab. Unaware of the hazard the employee reached into the vent to retrieve the pen. As the employee inserted their right index finger into the vent it came into contact with an unseen rotating blade inside resulting in a abrasion (skin was torn on the top and subsequent injury to the index finger.
