What happened
He was installing an actuator into the test fixture that requires a large pin to be threaded thru the top of the actuator thru the test fixture then another locking pin to be threaded thru the top of the larger pin to lock everything into place. He was stabilizing the other end of the actuator with his left hand while installing the locking pin his finger was pinched.
