Before the incident
Forklift operator ran over his own foot with the forklift. Narrative to be completed upon his completion of the incident report. Added [REDACTED] - [REDACTED] - EE reported to supervisor that he accidentally ran over his left foot when forklift slipped out of gear and or e brake did not work. Later was told story of EE reached into his pocket and accidentally dropped denture which landed on the ground. EE attempted to keep the forklift from hitting the denture and caused the forklift wheel to catch and run over the outer edge of left foot. EE was able to move toes and no broken bones were visible. Referred to [REDACTED] for emergent treatment. Returned with paperwork - no fracture. Restrictions - must wear boot and take OTC pain pills when needed. Return to check up in 1 wk ([REDACTED])
