What happened
There was a strap over the fork on the lift and an adjustable clamp on the end of the fork to keep the strap from sliding off the fork. The strap was attached to the plate with a clevis. While the plate was being twisted to fit to the unit the clamp on the fork shifted and allowed the strap to come off the fork. The plate came down a few feet and landed on his back.

The plate landed on his back. There were bolts welded to the plate that caused 2 gouges on his upper back. His back and left shoulder were injured.
