Before the incident
He was working out of a scissor lift 7 feet in the air welding on a unit. He was using a welder.
What happened
He was going to move the lift down and his ground clamp for the welder got stuck on the door causing the latch holding the door shut to malfunction. He reached for the ground clamp and the door came open. He reached for the gate and lost his balance causing him to fall out of the lift. The floor of the lift has a step down near the gate which is what caused him to lose his balance.
