Requested by admin & section Mgr to move cubical contents from [REDACTED] to [REDACTED]. Told to sort & clean up my cube in [REDACTED] under pressure from a different senior member of the team caused a need to clean the cubical before COB. Total time approx 3.5 hours
Injury or illness
Lower back pain. Pain increased on Friday Saturday severe back spasms inability to stand up without help. Sunday same. Contacted MDLIVE doctor on Sunday Prescription for a muscle relaxant cyclobenzaprine 5mg tablets. Able to walk with pain on Monday through to present.

Lifting boxes transporting boxes on a cart from H1765 to H1720. Filling cube in H1720. Sorting materials moved to H1720. Throwing away materials into a large dumpster. Attempt to move dumpster into the hallway for facilities to pick up (but was unable to clear the door opening.
