Placing patient breakfast tray on the bedside table to eat

I brought tray into patients roomset it on the table. I asked him if he was hungryhe said no. His urinal fell over when I pushed the table out the way. I went to pick it up and he grabbed my right arm and pulled me to him. He had restraints on but he could still grab me and he twisted my wrist. I screamed for help. Nobody could hear me. I got out of his grip and left the room. I told the nurse outside what happened. I had to go back in to feed the patient. I tried to feed him and he spit his food out at me. I left out again to finish my vitals with my other patients. [REDACTED] came to perform procedure at bedside and the nurse was trying to help. The nurse started yelling for me because he was kicking at her. I came to try and assist and he kicked me in the face.
