was assisting patient to bathroom she was a fall risk and unsteady on her feet pt was taken out of her 4 pt soft restraints and ambulated with [REDACTED] and myself on either side as she was unsteady and not easily directed Security was present Pt became aggressive and pushed me away setting her off balance Imaintained hands on contact to steady her from falling and she leaned over and bit my upper left bicep through my shirt saliva soaked through my sleeve onto my skin Pt escorted back to bed by security and [REDACTED] with restraints reapplied incident reported to chrge and house sup I sought care In ED per policy I dontremember the names of the witnesses
