Before the incident

Was in space I work carrying item and tripped over a rolled-up rug on floor.

What happened

Part of a rolled- up rug on floor due to moving and building new towers. I was walking and tripped over rolled up rug and twisted like a pretzel falling on my back and head hit on cement floor and arm cut on metal cabinet. head hard on cement floor screamed and then when I was able to get up from a tight little spot that I fell in I thanked [REDACTED] I was alive. In shock and very upset scared.

Head neck arm cut back arms ribs knees everything but my head injury is my main concern worried not internal bleeding inside.. Head hurt headache numb head and neck hurts arm cut ribs hurt back knees hurt everything hurts but my main concern is my head injury. Numbness on right side head and neck is extremely painful.

Object or substance involved

I was carrying a heater and tripped on rolled rug and fell in a little space cut arm on slide tower and hit head on cement floor landing on my back shoes fell off. Movers built towers and moved table around that and rug was rolled in order to build slide towers. I did put a facilinet order to cut rug. Coworker ended up cutting in after I fell to my assitance.
