What happened
Employee was removing the retaining plate frame from a bottom pour ladle. When they went to remove the bolts from the plate the frame did not come off so they used their hands to attempt to dislodge the frame from the ladle. The housing then dropped down causing a pinch to their left index finger. The left hand was placed in such a way that when the frame caught the edge of the support lip (the frame sits on) the lower part of their left index finger was caught between the two parts causing a laceration.
