What happened
Employee was driving a forklift having placed a load of material to organize it for inventory. When they drove away they attempted to press the brake but instead pressed the gas causing the forklift to strike a column at max speed on the side of the carriage. When the impact occurred their upper body went forward causing their head to strike the left handhold inside the cab causing a laceration to the scalp which required medical attention.
