Before the incident
Employee had jumped over a safety rail to get to a steam line valve to shut off. Used the city trough and city well as a short cut to get to the steam valve
What happened
After shutting off the steam valve used the short cut to come back to the plat form. After stepping onto a fiberglass grate laying over the city well reached up to grab onto a mid rail while trying to step onto the city drainage trough slipped and fell into the city well
