Before the incident
Reinstalling head-press tooling after greasing the mandrels on Line C12.
What happened
Rather than using the provided lift assist he attempted to manually lower the tooling into place. The tooling became stuck so he used a plug gage with his right hand to “tap” the tooling into place while his left hand rested on the top edge of the housing. When the tooling became unstuck and dropped down the overhang of the top edge of the tooling struck his left thumb.

The impact resulted in a fracture of the thumb and a laceration that required seven (7) stitches to close.
