What happened
[REDACTED] had just hooked up the 3 weight bars to a single point sling. The forklift was not center over the load and the load was unbalanced. He informed the forklift operator to lift the load without stepping out of the line of fire for if the tool was to swing. When the forklift operator lifted the load the tool string pivoted and swung into [REDACTED] right leg.
