Before the incident
The IE and IW installed centralizers on every other joint. According to the IW 's statement Casing was running smooth until the pup joint came up.
What happened
IE was holding the pup joint up while the IW was latching the elevator. The catwalk operator then skated joint during our process by bumping the controls causing pipe to skate forward hitting IE in the chest pining him to the stump until he twisted out and broke him self free.
