The EE also received four sutures. DESCRIPTIONAt approximately [REDACTED] the maintenance lead was called to line 3 to perform a tab adjustment. After locking out the equipment the EE took off the process lock of the lid. The EE raised the lid and simultaneously felt vinyl panel strike his back. The EE hand then turned around toward the material while placing the left hand hand on the frame of the [REDACTED] Form Roll(MRF). The lid came down causing a crushing the EEs left hand. The EE was transported to the local emergency room. ROOT CAUSELOTO procedure only identified two of the three present energy sources(pneumatic and electrical) not kinetic. There wasn 't a SOP or JSA for the task. CORRECTIVE ACTIONEngineered a lock out point for the lid of the MRF. Immediat
