Installing Conduit onto electrical wires

The maintenance employee was instructed to install a conduit over electrical wires under the new lifting platform installed in a pit-type area. EE lowered the lift platform and entered the pit by stepping onto the platform and entering through the gaps on the side of the pit see attached pictures. The EE went under the platform to install the conduit when the platform gave way and crushed the EE. Another maintenance employee noticed that when he called the injured employee on the radio there was no response. at this point there was a call on the handheld radios that the employee was not responding and all hands on deck went over to the incident area to find the EE trapped between the lift and platform. Maintenance personnel put straps on the lift and began removing the platform from the EE with a forklift. Once there was clearance to safely remove the EE was removed.
