Description of Occurrence*: [REDACTED] [REDACTED] (Electronic Report) I was walking to my car went out the outpatient surgery door twisted my ankle looked down and saw acorns i tried to catch my balance my lt foot rolled on the acorns and i fell hard to the street hit no left side. I was unable to get up due to extreme pain in my lt arm and neck. Another employee ran up and asked if i was ok i said no she said she would call her supervisor and she called a PI many people came EMS picked me up i had to go by stretch i still was unable to get up Activity Before Incident: i was in orientation had just clocked out was walking to my car
