What happened
Employee [REDACTED] was using glass crusher when he felt a piece of glass come up and land on his eye lid. Employee removed his glasses to brush the glass away and felt the glass enter his eye. Employee did not inform anyone at work. Employee went home and proceeded to try and clear out his eye himself. By flushing it while he was in the shower.
