Using an air wand to clean the top of the cutting table

[REDACTED] was working in cutting. He was blowing off the table switched the air want to his left non-dominant hand and continued to blow off the table. Once completed he was removing the wand and pointed the tip into the glass crusher. His index finger was under the trigger. As the crusher was pulling the wand in [REDACTED] decided to let go his finger was caught. This resulted in his index fingertip receiving an avulsion.
