The shipping receiving employee had just completed the loading of a trailer with totes. They shut the trailer doors then pressed the down button to close the overhead dock door (#8). The dock door did not lower all the way and the employee noticed that the cable reels were spinning (but the door was not lowering).

The employee made a decision to try and pull the door down by using a metal hook (which is used to adjust the dock leveler plate). To do this he stood underneath the open dock door. When he pulled at the door it unjammed and came down uncontrollably striking the employee on the top of the head causing a significant impact head laceration. The 911 emergency responders transported the employee to hospital where 11 sutures were used to close the wound.
