Before the incident
[REDACTED] was testing a valve and actuator that was mounted together. The actuator was the pneumatic type and the valve was a butterfly.
What happened
While testing he placed his thumb in the area where the valve seats. When he cycled the unit his thumb was caught between where the valve seats. The trauma was caused by the force of the pneumatic actuator driving the disc into the seat of the valve where his thumb was located
