What happened

I was at work for a meeting and after the meeting I was walking to the [R[REDACTED]ACT[REDACTED]] to drop off some of the left-over refreshments. I was in street clothing not a uniform I was wearing a pair of rubber Birkenstocks due to the inclement weather. While walking down the hall right after the locked doors to get into the [R[REDACTED]ACT[REDACTED]] from the Radiology area before Zone C right in front of the Security Guard there was a divot (actually two of them) in the flooring the toe of my left shoe got caught in the divot and I lost my footing and fell first to my left knee then to my right with much less force and then to my left side onto the floor breaking open a carton of fruit punch and becoming covered in it. It was very sticky. During this three-tiered fall I twisted my lower back a bit and it was a bit sore also. I was suddenly surrounded by MANY witnesses I do not have any of the names please check with security I believe they also put in a RL. This sudden stop and
