What happened
Patient was standing in doorway of [REDACTED] being asked to walked voluntarily into the [REDACTED] room which was planned to be utilized as a quiet room to relax Medical staff was standing inside the [REDACTED] anteroom and when the patient started walking into the room the patient drew back right upper extremity and hit medical staff in the head with a closed fist. The back of medical staff head also hit the wall behind her. Multiple security officers were behind the patient and at least 2 standing in the day area. Also Peds Psych Social worker was present at the time of this incident.
