De-accessing port

Was de-accessing patient port for failed blood return after port access. Patient needed CT contrast dye for test. Upon de-accessing port safety disk slid into place and 'clicked. ' Employee was placing needle into sharps container and protective disk slid back from its safety position as employee was placing needle component into sharps container. The safety disk got caught on the lip of the sharps container tilting the exposed needle up and punctured employee on the right index finger just below the nailbed.
