Before the incident
Assisted patient to bedside commode with use of walker
What happened
PT OT team transferred patient to bedside commode with RW. Patient walked 3 feet. Patient denied any dizziness while sitting on bedside commode initially. Patient began to c o dizziness. BP taken. Patient became unresponsive quickly slumped over to the right drooling. I assisted patient with elevating LE 's while my co-worker assisted patient from falling from commode. Patient did not respond more drooling. Code blue called. Total assist with nursing staff to lift patient over from commode to bed. repositioning patient in bed for improved comfort and safety. After the lift from commode to bed my back began hurting.
