Copied from [REDACTED] report form: (worker) was setting up a tank after CIP and the sample port jumper hose fell off the pipe fencing from above him. He was bent over so the reducer and flange hit the back of his head. He came to the office with a significant amount of blood on his shirt and paper towels compressed to the wound. He was given a clean towel to continue compression and a call was placed to IMT. (worker) was taken to the ER by an IMT member on shift
