Before the incident

Moving completed apron weldment to a staging area with an over head crane and hoists.

What happened

After moving the first apron weldment they didn 't adequately raise the hoists before moving the crane to pick up the next apron. They attached the rigging of one hoist to the next apron and then turned around to raise the other hoist which had a chain rigging suspended from the master hook. As they turn they turned into the chain rigging which struck their right incisor and chipped it.
