What happened
Employee had ensured the truck was secured and let the driver know it was secured to off load. As he was unloading the first pallet the driver pulled away from the dock while the employee was traveling across the dockplate casuing the dockplate and forklift to fall to the ground. Employee was seatbelted in the operators seat.
