operator was attempting to install the spout on a large conveyer. when installing it got stuck operator then used a flathead screw driver in attempt to pry the spout up. while doing so the screwdriver slipped off striking the operator under the right eye cutting it and poked the right eyeball as well causing it to bleed. operator went to see the nurse and then went into the ER
