Driving the reach truck
What happened
Employee was driving the reach truck down aisle 17 keeping an eye on the right rigger for clearance. He forgot to also keep an eye on the left rigger and caught it on a steel support pole.
Injury or illness
When the rigger caught on the steel support pole it caused the reach truck to stop. The employee 's body continued to move forward & employee hit their face on the plexiglass shield on the reach truck.
