What happened
Employee stepped on lower metal bar of rack of pick level A and with her left hand grabbed the third level metal bar (level C) to pull herself up to reach the top of a box. With her right hand she reached up to move the box to get an item inside. She reached the top of the box and started to pull the box forward. The box slid out of the pick location and onto the floor. Employee tried to prevent the box from falling and injured her right shoulder in doing so.
