What happened

[REDACTED] came into my office [REDACTED] at [REDACTED] and talked to me about an incident that happened to him [REDACTED]. He had not informed management prior to this conversation about the issue. I gave him the phone number to call Medcore right then and a claim was started with the reference #27722418. On [REDACTED] [REDACTED] was at Vtcu demoing a Bug-o track welder. [REDACTED] he noticed some irritation of his eyes. He put eye drops when he went to bed. Later woke up with eyes burning itchy and scratchy. Tried flushing his eyes out in the shower with cold water to no avail. He was unable to open his eyes due to the pain. At around [REDACTED] he had really bad pain and went into the er to receive treatment. The dr put in numbing drops and did an eye exam and also put in an antibiotic ointment. Numbing drops were administered just before being release. Went home and was able to sleep. The next day he got up and still had discomfort and sat around the house in a dim lit room. Was ab
